Job Duties & Responsibilities:

• Conducts comprehensive nutrition assessments across five key domains: food/nutrition history, anthropometric measurements, biochemical data, nutrition-focused physical findings, and client history.
• Identifies and documents nutrition diagnoses using standardized language (PES statements).
  Develops and implements nutrition care plans in collaboration with patients, caregivers, and the interdisciplinary team.
• Communicates and ensures the execution of nutrition plans with the team.
• Monitors and evaluates progress, adjusting interventions as needed.
• Adheres to the Standards of Practice (SOP) and Standards of Professional Performance (SOPP).
• Provides nutrition education to patients, families, and caregivers before discharge.
• Shares evidence-based nutrition information with colleagues and other healthcare professionals.

Mon-Fri 8am to 4:30pm

Salary:

$24.20-$38.70, based on work history/experience

Job Qualifications:

Bachelor’s degree in Food and Nutrition, Dietetics, Human Nutrition, or equivalent qualification is required.
Preferably 1+ years of previous clinical experience with patients across the aging continuum.
Must be board eligible for Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) by the Commission on Dietetic Registration (CDR); required within 3 months of hire.
Must obtain Licensed Medical Nutrition Therapist (LMNT) in the State of Nebraska within 3 months of hire at assigned facilities.
